TMS Institute named Bluegrass Advanced Mental Health its 2026 Top Ranked TMS Clinic in Louisville for transparency and treatment options.

TMS Institute, an independent national directory of TMS clinics, has named Bluegrass Advanced Mental Health its 2026 Top Ranked TMS Clinic in Louisville.

The recognition goes to the clinic with the highest TMS Institute directory score in each city with three or more listed clinics. BAMH ranked first among the three Louisville clinics in the directory. The score measures how much a clinic tells patients and how many treatment options it lists, checked against the clinic’s own website and public records. It is not a rating of care, results or patient satisfaction, and no clinic can pay for it.

“Most people think they know what OCD looks like: someone washing their hands until they crack, or checking a locked door a dozen times before leaving. What most people don’t see is the distress caused by the mental loop running underneath, and that’s usually the part that brings someone into my office.”

OCD is one of the most misunderstood diagnoses I evaluate, not because it’s rare, but because what it actually looks like rarely matches what people expect. Understanding how the OCD brain is wired differently is the first step toward getting the right kind of help.

          The cycle that drives OCD

Obsessive-compulsive disorder centers on a repeating loop: an intrusive, unwanted thought creates anxiety, a compulsive behavior temporarily relieves that anxiety, and the relief itself reinforces the cycle, making it more likely to happen again the next time an intrusive thought shows up.

That ”stuck” loop is the actual disorder. It’s not a personality issue or a behavior problem. OCD is a distressing, time-consuming thought, feeling, image, or compulsion that a person often recognizes as irrational even while unable to stop it. That insight, knowing the OCD doesn’t fully make sense but being unable to shake it anyway, is part of what makes it so exhausting and debilitating.

OCD often looks different than most people expect

The stereotype of OCD, visible handwashing or symmetry rituals, only describes a fraction of what I actually see. Common subtypes include:

Contamination OCD, fear of germs or illness, often but not always paired with visible washing or cleaning rituals

Harm OCD, intrusive fears of accidentally or intentionally hurting someone, despite having no actual desire to do so

Checking OCD, repeated checking of locks, appliances, or one’s own actions to prevent a feared outcome

Symmetry and “just right” OCD, distress until things feel arranged or done correctly

Intrusive taboo thoughts, unwanted, often disturbing thoughts about violence, sexuality, or religion that the person finds deeply distressing, not desirable

A significant portion of OCD compulsions are mental rather than visible: silent counting, mental reviewing, seeking reassurance, or repeating a phrase internally. That’s part of why it goes unrecognized so often, a theme I’ll cover more directly in the next piece in this series.

How OCD differs from a personality trait

Wanting things clean, organized, or done a certain way isn’t the same as having OCD, and I think this distinction gets lost constantly in everyday language. If a preference for order is generally consistent with who someone is and it doesn’t cause significant distress, then it is a personality trait. OCD is different: it’s intrusive, unwanted, often distressing. Clinically, OCD is defined as taking more than an hour a day or causing significant distress or impairment, and for many people it takes far more and can greatly interfere with their ability to function at work, in relationships, and at school.

          How I evaluate OCD

A proper evaluation goes beyond asking whether someone washes their hands a lot. I assess the actual cycle: what the intrusive thought is, what compulsion follows, how much time it consumes, and how much it interferes with daily life. We use a structured tool called the Yale-Brown Obsessive Compulsive Scale (Y-BOCS) to assess a patient’s symptom severity and track treatment progress over time.

Getting this evaluation right matters enormously, because OCD is frequently misdiagnosed as generalized anxiety, ADHD, depression, oppositional behavior, or in some cases even a psychotic disorder, especially when the compulsions are mental rather than visible. An inaccurate diagnosis leads to the wrong treatment, which wastes time that could have gone toward something that actually helps.

          What comes after the diagnosis

Once we’ve confirmed OCD and measured its severity, treatment usually starts with exposure and response prevention (ERP), a specialized form of cognitive behavioral therapy, often paired with medication such as an SSRI. For adults whose symptoms persist despite those first-line treatments, we offer Deep TMS, which is FDA-cleared for OCD and works best alongside ERP. We coordinate with ERP-trained therapists across Louisville, so patients don’t have to choose between their therapist and our clinic. You can read more about

          Key takeaways

OCD is driven by a specific cycle: intrusive thought, anxiety, compulsion, temporary relief, reinforcement

Most OCD doesn’t look like the stereotype; contamination, harm, checking, symmetry, and intrusive taboo thoughts are all common subtypes

Many compulsions are mental, not visible, which is a major reason OCD goes unrecognized

OCD is distinct from a personality preference for order

An accurate diagnosis, using tools like the Y-BOCS, is the necessary first step toward effective treatment

Is OCD just about being very clean or organized?

No. Cleanliness and organization can be part of certain OCD subtypes, but OCD itself is defined by the intrusive-thought-and-compulsion cycle, not by any specific theme. Many people with OCD have subtypes that have nothing to do with cleanliness at all.

Yes. Mental compulsions, like silent counting, reviewing, or reassurance-seeking, are just as real as visible ones and are often harder for others, and sometimes even the person experiencing them, to recognize as OCD.

Most people start with exposure and response prevention (ERP) therapy, medication, or both. For adults who still have significant symptoms after those treatments,

Can You Do TMS While Taking Antidepressants?

Yes, you can usually stay on your antidepressant during TMS. Dr. David Roth explains how the two work together and when adjustments make sense.

"Short answer: yes. Taking antidepressants doesn’t disqualify you from TMS, and for many patients, the two are used together as part of the same treatment plan. Here’s what that actually looks like, and when a different approach might be recommended instead."

One of the most common questions before starting TMS is whether it means stopping current medication first. It doesn’t, and understanding why helps explain

how TMS actually fits into a broader treatment plan rather than replacing one entirely

          .

Why the two aren’t mutually exclusive

TMS and antidepressants work through different mechanisms. Antidepressants adjust neurotransmitter activity systemically, throughout the brain and body.

          TMS delivers targeted magnetic stimulation

to specific brain circuits involved in mood regulation, without circulating through the bloodstream. Because they act on different systems, using both together is a normal part of care, not a conflict to manage. Your provider will also review any medications that can affect TMS safety or response, such as those that lower the seizure threshold, and adjust your plan if needed.

What the clinicians at Bluegrass Advanced Mental Health evaluate before recommending combined treatment

During your evaluation, the clinicians at Bluegrass Advanced Mental Health review:

Your current medication, dosage, and how long you’ve been on it

Your response so far, partial improvement, no improvement, or side effects that haven’t resolved

Whether your symptoms suggest TMS alone, TMS alongside your current medication, or a medication adjustment first

This isn’t a one-size-fits-all decision. Some patients continue their exact current regimen throughout TMS treatment; others make small adjustments alongside starting TMS, based on provider's clinical judgment.

When a taper might be recommended instead

In some cases, particularly when a current medication isn’t providing benefit and has significant side effects, the clinicians at Bluegrass Advanced Mental Health may recommend

adjusting or tapering it as part of your overall plan

. This is a case-by-case clinical decision, not a default requirement of starting TMS.

Key takeaways

Taking antidepressants does not disqualify you from TMS at Bluegrass

TMS and antidepressants work through different mechanisms, which is why combining them is common

The clinicians at BAMH evaluate your specific medication history and response before recommending whether to continue, adjust, or combine treatments

Any medication changes are a clinical decision made with your provider, not a requirement to start TMS

Do I need to stop my antidepressant before starting TMS?

No. Most patients continue their current medication throughout TMS treatment. Any changes are made based on your individual evaluation with your provider.

Will combining TMS and medication make either one less effective?

There’s no indication that combining the two reduces effectiveness. Many patients use both as part of the same treatment plan specifically because they work through different mechanisms.

That’s actually one of the most common reasons patients seek out TMS in the first place. The clinicians at Bluegrass Advanced Mental Health will discuss whether to continue, adjust, or taper your current medication as part of your overall plan.

The honest answer is that TMS cost depends almost entirely on your insurance, not on the treatment itself. Most major plans, including Aetna, Anthem, Cigna, Medicare, Tricare, and UnitedHealthcare, cover TMS for treatment-resistant depression when certain criteria are met. The treatment itself doesn’t have one fixed price. What varies is whether you’re paying in-network rates, out-of-network rates, or self-pay.

Not everyone has in-network coverage, and some patients specifically choose accelerated TMS for scheduling reasons even when it changes their coverage picture. For those situations, third-party financing options may be available to reduce the upfront cost. The goal is the same either way: no surprise bills, and a full cost conversation before treatment starts, not after.

Does insurance cover accelerated TMS the same way it covers standard TMS?

Accelerated TMS is typically not covered by insurance at this time, as most insurers only reimburse the standard once-daily protocol. The all-inclusive cost is $7,500, which includes your evaluation, 38 sessions during treatment weeks, all follow-up visits, and free touch-up sessions within 6 months if needed. FSA dollars are accepted, and CareCredit financing may be available for eligible patients. We understand this is a significant investment, and your written cost estimate comes before anything is scheduled. Many patients find the shorter time commitment worth the cost.

How TMS works, what a session feels like, and Standard vs. Accelerated options, medically reviewed by Dr. David Roth. Louisville, KY.

"If antidepressants haven’t given you your life back, TMS offers a different path. Instead of medication that circulates through your whole body, TMS uses targeted magnetic pulses to stimulate the specific brain circuits involved in mood regulation. No pills, no sedation, no systemic side effects, and for many people with treatment-resistant depression, real relief when other approaches haven’t worked."

If you’re reading this, you’ve probably already tried more than one antidepressant. Maybe the first one did nothing. Maybe the second one worked for a while and then stopped. Maybe the side effects, the weight changes, the emotional flatness, the fatigue, cost you as much as the depression did. That frustration is exactly why TMS exists as an option.

to the brain regions that regulate mood, most often the left dorsolateral prefrontal cortex. In people with depression, this region often shows lower activity than in people without depression. Repeated stimulation over a full treatment course can help restore healthier activity in that circuit.

The clinicians at BAMH will help you determine which protocol fits your situation during your evaluation. Some patients start standard and later choose accelerated for a future course; others go straight to accelerated because of scheduling. There’s no wrong choice, only the one that fits your life.

Who’s a good candidate for TMS?

TMS is generally considered for adults and adolescents (ages 15 and older) at Bluegrass AMH with moderate to severe depression that hasn’t responded adequately to medication. Signs it may be worth discussing with Dr. Roth include:

You’ve tried one or more antidepressants without enough relief

Side effects from medication have been difficult to tolerate

You want a non-medication option that can be used alone or alongside your current treatment

You can commit to the treatment schedule, standard or accelerated

Who may not be a good candidate

TMS isn’t right for everyone. It may not be recommended if you have certain non-removable metal implants or devices near the head (like cochlear implants or aneurysm clips), an active substance use disorder, or other conditions that could affect safety. Dr. Roth reviews your full medical history during your evaluation to determine whether TMS is appropriate.

Whether you choose standard or accelerated TMS, the in-session experience is the same:

You can listen to music, watch relaxing videos, read, meditate, or simply rest during your session

You’ll feel a light tapping sensation on your scalp and hear a clicking sound as the pulses are delivered

TMS is generally well tolerated. The most common side effects include mild scalp discomfort or tenderness (most noticeable in the first few sessions), headache, or temporary lightheadedness. Serious side effects, including seizure, are rare. Dr. Roth reviews your medical history before treatment begins to confirm TMS is a safe option for you.
